Pilot error in landing in thick fog is blamed who tried to land 3 times and fatally failed on last 4th landing attempt. Polish President Lech Kaczynski and his wife Maria Kaczynski were on board the plane and have been killed.along with Katyn memorial governement delegation of 96 on the way to Smolensk.
