Biagio Nazzaro was killed during the XVI Grand Prix de l'Automobile Club de France, held on Saturday, 15 July 1922 at Strasbourg, Bas-Rhin department in north-eastern France.

About five hours after the start of the 60-lap race, while following the leading group, Biagio Nazzaro’s Fiat 804 had a rear axle problem at full speed. It threw a rear wheel, went out of control, hit a tree and rolled over several times. The accident occurred about half a mile from the Eintzheim hairpin, along the straightaway towards Innenheim, on 52nd lap. Different accounts indicated it was the 57th lap.

Biagio Nazzaro and his riding mechanic Felice Germano were taken to Strasbourg hospital where Nazzaro died shortly afterwards. Some sources reported that also his riding mechanic succumbed to his head injuries in the following days. This is not true, in an article on Biagio Nazzaro's funeral, published by the Italian newspaper La Stampa, dated 27 July 1922, there is a statement from Germano's mother confirming that her son was still alive at the time. Information on Germano's death is therefore very likely wrong.
