A report into the 2020 derailment of a passenger train travelling from Sydney to Melbourne has found no protocols were in place to ensure the driver had understood important changes to his route. The Australian transport safety bureau says a hut fire two weeks earlier meant signalling was down and drivers were relying on paper instructions.
