I just got a letter from Providence Health saying my plan would be converted from 1000 Gold to 2500 Silver if I "chose to do nothing" by the end of the year.
Of course, when I picked the plan - it wasn't even listed on their website ( was only listed on healthcare.gov marketplace ) and I called to ask if it was being discontinued at the end of the year since I can't find it on their website.
This is very common. They re-name the plans, change the coverage, etc. The plans also get re-negotiated with national hospital groups as to what they are willing to accept, what is considered "usual and customary" fees/prices, etc.
It would be a lot easier if they sent out a letter saying "Due to constantly changing costs, variables, and contract negotiations - You can have this exact same coverage, but the price will be $$$ starting 1/1/2017 ... If you would like a cheaper plan or different coverage, please visit our website or healthcare.gov and make a choice before 12/15/2016"

Maybe 20-30% ish get that, but in a lot of cases the vagina tears on it's own. Most doctors practicing evidence based medicine do not do episiotomies anymore. It was proven to provide no benefit for the mother/child.
The last time I did research on this was about a year ago, and my understanding from the available information is that if the doctor thinks you will tear naturally, they will cut it because a cut will heal faster than a tear.
