Police said a suicide bomber rushed towards people as they were leaving church in the Pakistani city of Peshawar.
He blew himself up near the gates of the building in a densely populated area close to a market.
Local authorities say at least 40 were killed.
The victims included a number of children.
At least 70 more have been wounded.
Christians make up about four per cent of the country's 180 million population.
They tend to keep a low profile because of the risk of attack by Islamist militants who often bomb those they view as heretical.
